Document Collection by Proxy

There are a number of documents that are made available for collection from Ask 911±¬ÁÏÍø, and there are often times when the owner of these is unable to collect the documents themselves.

In these cases, someone may authorise a third party (proxy) to collect the documents on their behalf. Proxy authorisation can be made at the time of placing your request (via the Online Shop for transcripts and letters, or via My911±¬ÁÏÍø for testamurs) but in the event that this has not occurred the below Third Party Proxy Authorisation Form can be submitted.

If you have been notified that one (or more) of the below documents have become available for collection from Ask 911±¬ÁÏÍø, but are unable to collect it yourself, this form has been provided to nominate a proxy to collect them on your behalf:

  • Letter
  • Testamur
  • Transcript
